Thank you for sending that information. We sent it on to the MFA to approve your entry to Israel. However, they said that in order to complete the process, they need to receive the health statement form that we sent below asap. You can also access it here.


Please let me know when you have it filled out. If possible, I will appreciate if you can send me a copy of the form so we can expedite the process.

Stay for the quarantine.
