When you arrive at the Tel Aviv Branch Office, take one # under Report of Birth.

What to bring to the appointment for Caroline

1. Both Parents must be present (both parents need to bring a valid US/Israeli passport)
2. Child must attend appointment
3. DS-11(Both sides completed and unsigned)
4. 5*5cm photo, with white background and no glasses!
5. DS-2029 (Bring the whole packet (7 pages) completed pages 1&2 with all physical presence listed (#24 and #25)
6. Flight Itinerary( if flying within 2-3 weeks)
7. Divorce Decree (if any parent has been divorced)
8. Originals and a set of three photocopies for each of the baby’s birth certificate, parents’ valid and signed passports and one copy of parent’s marriage certificate. **if these documents are NOT in English, please translate them using the attached translation sheet.
9. Name change document (if names were changed)

10.Documents and proof of your physical residence in the U.S. (school transcripts, pay slips, phone/electric bills, rental agreements etc)

11.Fee: https://il.usembassy.gov/u-s-citizen-services/child-family-matters/birth/fees/

12. Social Security form(completed and signed by a parent):see attached

Please come to the Embassy Branch Office – Tel Aviv on Thursday, April 25th at 9:15am. Please bring this e-mail to facilitate your entrance to the Tel Aviv Branch Office.
